As part of the registration process, we require your consent
to the following terms and conditions regarding your
participation in this course.

Please be advised that the course registration fee is
non-refundable under any circumstances. By signing this
consent letter, you acknowledge and agree to this policy.
3. Participation and Commitment
By enrolling in this course, you agree to attend all
scheduled sessions and complete all required coursework
to the best of your ability. You understand that failure
to do so may result in the forfeiture of your course fee
and/or the inability to complete the course.
4. Waiver of Liability
You hereby release and hold harmless Esthetica Dental Academy,
its staff, instructors, and affiliates from any liability,
claims, or damages that may arise from your participation
in this course.
5. Consent to Photography/Videography
By signing below, you consent to the use of photographs
or videos taken during the course for promotional or
educational purposes by Esthetica Dental Academy.
6. Confidentiality
You agree to maintain the confidentiality of any proprietary
information shared during the course and not to disclose
any such information to third parties without the express
written consent of Esthetica Dental Academy.
